Dr. Arif Nazir, president of SHC Medical Partners, presented the company’s innovative Care Hub Model for long-term care at the 2022 AMDA PALTC annual conference in Baltimore, Md., on March 12th. He also was featured during AMDA’s Drive to Deprescribe (D2D) campaign session.
AMDA’s Society for Post-Acute and Long-Term Care Medicine (PALTC) represents a community of over 50,000 medical directors, physicians, nurse practitioners, physician assistants, and other practitioners working in the various post-acute and long-term care (PALTC) settings. The annual conference is designed to promote and enhance the development of competent, compassionate, and committed medical practitioners and leaders to provide goal-centered care across all post-acute and long-term care settings.
