As a practicing post-acute care geriatrician for over 15 years, Dr. Nazir recognized the frontline barriers to high quality care coordination and focused his research on finding practical solutions. This passion led him to the innovative strategy behind the CARE Hub model.
He also led the OPTIMISTIC project for the Center for Medicare and Medicaid Services Innovations. This initiative utilized trained practitioner teams and evidence-based principles for proactive care delivery and care navigation to provide the highest quality outcomes and resulted in improving care for thousands of patients in Indiana. He brings years of experience and proven care concepts to the Care Hub approach. The results so far have been phenomenal.
As a national leader in the post-acute care setting, Dr. Nazir serves as the President-elect and Chair of Innovations for the Society of Post-Acute and Long-Term Care Medicine (paltc.org). He has published his research in several peer-reviewed journals and frequently presents at regional and national platforms on geriatric care issues.
