Dear Colleagues,
Thank you for all your hard work and courage that is at display during these testing times for the country, and specifically for our patients, families and healthcare teams in the post-acute care frontlines.
True geriatrics care is what is going to help us succeed through this pandemic. We encourage you to focus on all elements of interdisciplinary care including:
- Deprescribing of medications
- Reduce unnecessary testing and consultations
- Focusing on advance care planning—all elements of interdisciplinary care
Over the last few weeks, many of my colleagues across the country have shared best practices:
- Best infection control outcomes can be achieved by following the set protocols and policies:
- Daily huddles and check-ins with your facility leaders and staff, improve communication and teamwork
- Prevalence of COVID patients continues to significantly lower than other national hot spots (New York, Seattle etc.) and avoidable hospitalizations can be prevented by sticking to in-depth assessments and intense
- Non-COVID respiratory illnesses continue to be the main cause of co-morbidity among nursing home patients

- To prepare for the potential surge, hospitals seek our partnerships through channels of communication and processes to efficiently transfer patients that are COVID negative.
