We are hiring for these positions across our network:


Nurse Practitioner

As a Nurse Practitioner with SHC Medical Partners, you’ll help with all aspects of patient clinical care, including diagnosis, treatments and consultations. You may also conduct physicals, order tests and serve as a patient’s primary healthcare provider. 

Our NPs perform both independently and in collaboration with the facilities medical director, and other care collaborative providers, to provide care and compassionate treatment for our residents.  You’ll be part of a team of that truly change the outcome of people’s lives!

What we require:

  • Successful completion of an approved Nurse Practitioner (MSN) or Physician Assistant Program
  • Maintain in good standing a current national and state licensure as an APRN and RN in the state for which you have applied
  • Must have a current/active CPR certification

Psychiatric Mental Healthcare Nurse Practitioner:

Our PMHNPs provide integrated medical and psychiatric care for residents and patients. As a PMHNP, you will have the opportunity to participate in cutting-edge care models, ongoing learning and meaningful data for self-improvement and best possible clinical outcomes.  A key member of the Care Hub program, you will work with the Care Hub medical director, medical nurse practitioners, lead psychiatrist, social workers, care navigators and facility staff to provide much needed psychiatric services. 

What we require:

  • Currently licensed PMHNPs who have one year of experience as Nurse Practitioners (APRN, PA)
  • Maintain in good standing a current national and state licensure as a PMHNP/APRN and RN in the state for which you have applied
  • Must have a current/active CPR certification

Licensed Clinical Social Worker

Our Licensed Clinical Social Workers provide screenings, clinical assessments, treatment, case management, consultation, referrals and treatment of emotional and behavioral problems. LCSWs also coach residents on how to manage ongoing health conditions and provide therapy to help patients make positive behavior changes.

What we require:

  • Current unrestricted certificate of licensure as a LCSW to work independently without clinical supervision
  • Experience in Mental Health, Behavioral, or Long Term Care with assessment and counseling skills
  • Must have a current/active CPR certification

Care Navigator:

Our Care Navigators provide high level customer service as part of our healthcare team.  They serve as a single point of contact for our medical teams, tracking and coordinating care delivery. We are searching for someone that has experience in data entry, process management and tracking, strong computer and communication skills, the ability to work independently and that has previous healthcare experience.

What we require:

  • High School Diploma required, advanced education in healthcare preferred.
  • Minimum of two (2) years experience in long term care preferred
  • Understanding of healthcare systems, EHR, healthcare operations/clinical workflows
  • Demonstrated intermediate to advanced skills in Microsoft Word, Excel and Outlook, and Internet/Intranet navigation

Billing Associate:

Our dedicated Billing Associates support our business by providing high quality billing and customer service as part of our healthcare team. They are responsible for our monthly insurance billings, including obtaining and submitting billing in support of our medical professionals.  As part of our Revenue Cycle Team, they work with all other disciplines and departments to provide superior support.

What we require:

  • High School Diploma required, equivalent related work experience preferred.
  • Minimum of one (1) year experience in billing and/or coding, preferred
  • Understanding of healthcare financial/billing systems, EHR, healthcare operations/clinical workflows.
  • Demonstrated intermediate to advanced skills in Microsoft Word, Excel and Outlook, and Internet/Intranet Navigation
